Individual Factors Associated with the Consumption of Illicit Substances: a Review of Reviews

Introduction: According to national studies, the use of illicit drugs is growing in Colombia. With this, the prevalence of substance use disorders and the set of health effects related to this practice also increases. Knowledge of the factors associated with the use of illicit drugs is necessary to guide the comprehensive care of the phenomenon. Methods: This is a systematic review of reviews on factors associated with the consumption of illicit drugs with seven databases and evaluation of the quality of the manuscripts according to AMSTAR. Results: Information was extracted from 38 reviews on individual factors associated with the use of illicit drugs. Demographic factors are associated with consumption through other factors. There is evidence of the association between mental and behavioural disorders and personality disorders. Conclusions: The likelihood of consumption of illicit substances and disorders due to their use is affected by a set of personal factors including sociodemographic characteristics, mental health conditions, sexual behaviour, legal drug use, age of onset and risk perception.

Fitness Changes in Adolescent Girls Following In-School Combined Aerobic and Resistance Exercise: Interaction With Birthweight

Purpose: To assess the efficacy of a supervised in-school combined resistance and aerobic training program in adolescent girls and investigate whether responses differ according to birthweight. Methods: Participants (girls aged 13–17 y) were randomized either to an intervention replacing physical education (PE) classes with 2 × 60-minute training sessions per week (n = 58) or to a control group that continued to attend 2 × 60 minutes per week of curriculum PE (n = 41). We measured muscular fitness (handgrip, standing long jump, and sit-ups), cardiorespiratory fitness (20-m shuttle run), skinfolds, and lean body mass preintervention and postintervention and determined effect size (Hedge’s g) differences between changes in these measures. We also compared changes within lower (<3000 g) and normal birthweight intervention and PE control subgroups. Results: The intervention group showed greater improvements in all the fitness measures and lean body mass (g = 0.22–0.48) and lower skinfold increases (g = 0.41) than PE controls. Within the intervention group, improvements in all fitness measures were larger in lower birthweight (g = 0.53–0.94) than in normal birthweight girls (g = 0.02–0.39). Conclusion: Replacing curriculum PE with supervised training improved muscular and cardiorespiratory fitness and body composition outcomes in adolescent females. Our findings suggest an enhanced adaptive response to training in participants with lower birthweight which warrants further investigation.

Who is meeting the strengthening physical activity guidelines by definition: A cross-sectional study of 253 423 English adults?

The current UK physical activity guidelines recommend that adults aged 19 to 65 years perform activity to strengthen muscle and bone a minimum of twice weekly. The number of adults meeting strengthening activity guidelines is lower than for aerobic activity, but estimates vary between studies partly due to differences in how muscle-strengthening activity is defined. We aimed to provide estimates for strengthening activity prevalence in English adults based on a nationally representative sample of n = 253,423 18-65-year-olds. We attempted to quantify the variation in estimates attributable to differences in the way strengthening activity is defined. Finally, we aim to provide a brief descriptive epidemiology of the factors associated with strengthening activity. Adults met guidelines for aerobic activity if they reported the activity equivalent to >150 min/week moderate-intensity exercise. Respondents met strengthening guidelines if they reported at least two bouts per week of strengthening activity. We defined strengthening activity, first, according to criteria used in the Health Survey for England (HSE). Second, we counted bouts of strengthening activities for which we could find evidence of health-related benefits (Evidence). Third, we included bouts of strengthening activity as defined in current UK physical activity guidelines (Guideline). Two-thirds (67%) of adults met guidelines for aerobic activity (69% of men, 65% of women). Less than one-third (29% of men and 24% of women) met guidelines for the HSE definition of strengthening activity. Under the Evidence definition, 16% of men and 9% of women met strengthening guidelines. Using the most-stringent definition (Guideline) just 7.3% of men and 4.1% of women achieved the recommendations for strengthening activity. We found females and older adults (50–65 years) were less likely to meet guidelines for aerobic, strengthening, and combined aerobic plus strengthening activity. The prevalence of meeting activity guidelines was lower in adults from more deprived areas (compared with the least deprived); Adults with lower academic qualifications (Level 1) were less likely to meet activity guidelines than those educated to Level 4 (Degree Level) or higher. Having a limiting disability was associated with a lower prevalence of meeting activity guidelines. Associations between socio-demographic measures and the prevalence of adults meeting activity guidelines were stronger for strengthening activity than for aerobic 51(or combined aerobic plus strengthening) activity Compared with aerobic activity, fewer adults engage in strengthening activity regardless of how it is defined. The range in estimates for how many adults meet strengthening activity guidelines can be explained by variations in the definition of ‘strengthening’ that are used and the specific sports or activities identified as strengthening exercise. When strengthening activity is included, the proportion of English adults meeting current physical activity guidelines could be as high as 1 in 3 but possibly as low as just 1 in 20. A harmonized definition of strengthening activity, that is aligned with physical activity guidelines, is required to provide realistic and comparable prevalence estimates.

Anger or emotional upset and heavy physical exertion as triggers of stroke: The INTERSTROKE study

Aims: In INTERSTROKE, we explored the association of anger or emotional upset and heavy physical exertion with acute stroke, to determine the importance of triggers in a large, international population. Methods and results: INTERSTROKE was a case-control study of first stroke in 32 countries. Using 13 462 cases of acute stroke we adopted a case-crossover approach to determine whether a trigger within 1 hour of symptom onset (case period), vs. the same time on the previous day (control period), was associated with acute stroke. A total of 9.2% (n = 1233) were angry or emotional upset and 5.3% (n = 708) engaged in heavy physical exertion during the case period. Anger or emotional upset in the case period was associated with increased odds of all stroke [odds ratio (OR) 1.37, 99% confidence interval (CI), 1.15-1.64], ischaemic stroke (OR 1.22, 99% CI, 1.00-1.49), and intracerebral haemorrhage (ICH) (OR 2.05, 99% CI 1.40-2.99). Heavy physical exertion in the case period was associated with increased odds of ICH (OR 1.62, 99% CI 1.03-2.55) but not with all stroke or ischaemic stroke. There was no modifying effect by region, prior cardiovascular disease, risk factors, cardiovascular medications, time, or day of symptom onset. Compared with exposure to neither trigger during the control period, the odds of stroke associated with exposure to both triggers were not additive. Conclusion: Acute anger or emotional upset was associated with the onset of all stroke, ischaemic stroke, and ICH, while acute heavy physical exertion was associated with ICH only.

Fructose-1,6 Bisphosphatase Deficiency: Cause of Severe Hypoglycemia in Young Adults

Fructose-1,6-bisphosphatase deficiency (FBPase deficiency) is a rare congenital metabolic defect affecting gluconeogenesis. It is an autosomal recessive genetic disease. The patient presents with fasting hypoglycemia and metabolic acidosis, and may have hyperventilation, apnea, hypoglycemia, and ketosis. Although the disease can be fatal in the neonatal period, appropriate treatment can produce an excellent prognosis. Here we present the case of a 21-year-old patient with fructose-1,6-bisphosphatase deficiency, who presented with viral gastroenteritis that caused decompensation of her underlying pathology, the patient presented satisfactory evolution with crystalloids and intravenous dextrose. This case is presented because of its low frequency, with few reports in adults and with adequate response to dietary treatment.

Peroxisome proliferator-activated receptor delta-PPARδ agonist (L-165041) enhances bovine embryo survival and post vitrification viability

The effect of L-165041 (PPARδ-agonist) on decreasing apoptosis and intracellular lipid content was assessed in fresh and vitrified-warmed in vitro-produced bovine embryos. It was hypothesised that the addition of L-165041 to the culture medium enhances development and cryopreservation. Oocytes were allocated to one of two treatments: control-standard culture medium, or L-165041 added to the medium on day 1 with no media change. Ultrastructure, cleavage, and blastocyst rates were evaluated in fresh, and in post-vitrification cultured embryos by optical and electronic microscopy. A subset of fresh embryos were fixed for TUNEL assay and for Sudan-Black-B histochemical staining. Vitrified-warmed embryos were assessed using MALDI-MS technique. Cleavage and blastocyst rates (control 49.4 ± 5.2, L-165041 51.8 ± 4.3) were not influenced by L-165041. The proportion of inner cell mass cells (ICM) was higher in fresh embryos, and the rate of total and ICM apoptosis was lower in L-165041. In warmed-embryos, total and ICM apoptosis was lower in L-165041. The overall hatching rate was higher in L-165041 (66.62 ± 2.83% vs 53.19 ± 2.90%). There was less lipid accumulation in fresh L-165041-embryos. In conclusion, the use of L-165041 is recommended to improve the viability of in vitro-derived bovine embryos.

Popliteal artery pseudoaneurysm caused by non–penetrating trauma in a patient with hereditary multiple osteochondromatosis

Hereditary multiple osteochondromatosis is a genetic condition characterized by the appearance of numerous osteochondromas, which can cause pseudoaneurysms in rare cases. The following article describes a 15-year-old patient with a history of current massages as part of his gym routine, who arrived at the emergency department with 4 days of pain, and ecchymosis in the right popliteal region. Therefore, duplex ultrasonography and arteriography were performed, confirming the diagnosis of popliteal pseudoaneurysm, which was subsequently treated by open surgery, providing a satisfactory outcome.

Psychometric performance of the brief Francis scale of attitude towards Christianity (Francis-4) in Mexican university students

The brief Francis scale of attitude towards Christianity (Francis-4) psychometric performance in Mexico is unknown. The objective was to evaluate the dimensionality, homogeneity, gender differential item functioning (DIF), and nomological validity of Francis-4. 1,108 university students between 18 and 60 years of age were evaluated; 80.4% were women (n = 891). CFA was performed with the following fit indicators: chi-square, RMSEA, CFI, TLI, and McDonald’s omega were calculated. For nomological validity, the mean and standard deviation according to gender were compared with Student’s t-test. Gender DIF was quantified with Kendall’s tau-b. The CFA revealed the following goodness-of-fit coefficients: chi-square = 25.45, df = 2, p = .001, RMSEA = .10, 90% CI.07 –.14, CFI = .99, TLI = .99, and SRMR = .01. Cronbach’s alpha of.97 and McDonald’s omega were.97. Scores were significantly higher in women than in men (M = 8.9, SD = 5.4 versus M = 7.6, t = 3.15, p = .002). Kendall’s tau ranged from.07 to.09 indicating absence of gender DIF. Francis-4 is a unidimensional instrument with nomological validity, high internal consistency, and absence of gender DIF. It is recommended to evaluate its performance in other populations.

Validity of the Hospital Anxiety and Depression Scale (HADS) in primary care patients in Colombia

Objective: To assess the validity of the Hospital Anxiety and Depression Scale (HADS) for depression and anxiety screening in primary care patients in Colombia. Methods: A criterion validity study was conducted with 243 adults that had completed the HADS and were later assessed using the MINI as a gold standard. Cronbach\'s alpha, McDonald\'s omega and factor structure were applied through confirmatory factor analysis (CFA). ROC curve analysis and Youden\'s statistic were used to determine the cut-off point. Results: Cronbach\'s α was reported to be 0.85 and 0.82 for McDonalds\' ω. CFA supported a two-factor solution demonstrating satisfactory fit. Root mean square error of approximation = 0.04, Comparative Fix Index (CFI) and Tucker-Lewis Index (TLI) = 0.97. For HADS-A, the cut-off point was determined as 6 associated with a sensitivity of 0.76, a specificity of 0.72 and Youden\'s index of 0.50. The ABC was 0.81. For HADS-D, the cut-off point was determined as 4 associated with a sensitivity of 0.78, a specificity of 0.74 and Youden\'s index of 0.53. The ABC was 0.82. Conclusion: The HADS is a valid and reliable instrument for anxiety and depression screening in adult patients of primary healthcare services in Colombia.

Antiproliferative, Antiangiogenic, and Antimetastatic Therapy Response by Mangiferin in a Syngeneic Immunocompetent Colorectal Cancer Mouse Model Involves Changes in Mitochondrial Energy Metabolism

In spite of the current advances and achievements in cancer treatments, colorectal cancer (CRC) persists as one of the most prevalent and deadly tumor types in both men and women worldwide. Drug resistance, adverse side effects and high rate of angiogenesis, metastasis and tumor relapse remain one of the greatest challenges in long-term management of CRC and urges need for new leads of anticancer drugs. We demonstrate that CRC treatment with the phytopharmaceutical mangiferin (MGF), a glucosylxanthone present in Mango tree stem bark and leaves (Mangifera Indica L.), induces dose-dependent tumor regression and decreases lung metastasis in a syngeneic immunocompetent allograft mouse model of murine CT26 colon carcinoma, which increases overall survival of mice. Antimetastatic and antiangiogenic MGF effects could be further validated in a wound healing in vitro model in human HT29 cells and in a matrigel plug implant mouse model. Interestingly, transcriptome pathway enrichment analysis demonstrates that MGF inhibits tumor growth, metastasis and angiogenesis by multi-targeting of mitochondrial oxidoreductase and fatty acid β-oxidation metabolism, PPAR, SIRT, NFκB, Stat3, HIF, Wnt and GP6 signaling pathways. MGF effects on fatty acid β-oxidation metabolism and carnitine palmitoyltransferase 1 (CPT1) protein expression could be further confirmed in vitro in human HT29 colon cells. In conclusion, antitumor, antiangiogenic and antimetastatic effects of MGF treatment hold promise to reduce adverse toxicity and to mitigate therapeutic outcome of colorectal cancer treatment by targeting mitochondrial energy metabolism in the tumor microenvironment.

Computational study, synthesis and evaluation of active peptides derived from Parasporin-2 and spike protein from Alphacoronavirus against colorectal cancer cells

Parasporin-2Aa1 (PS2Aa1) is a toxic protein of 37 KDa (30 kDa, activated form produced by proteolysis) that was shown to be cytotoxic against specific human cancer cells, although its mechanism of action has not been elucidated yet. In order to study the role of some native peptide fragments of proteins on anticancer activity, here we investigated the cytotoxic effect of peptide fragments from domain-1 of PS2Aa1 and one of the loops present in the binding region of the virus spike protein from Alphacoronavirus (HCoV-229E), the latter according to scientific reports, who showed interaction with the human APN (h-APN) receptor, evidence corroborated through computational simulations, and thus being possible active against colon cancer cells. Peptides namely P264-G274, Loop1-PS2Aa, and Loop2-PS2Aa were synthesized using the Fmoc solid-phase synthesis and characterized by mass spectrometry (MS). Additionally, one region from loop 1 of HCoV-229E, Loop1-HCoV-229E, was also synthesized and characterized. The A4W-GGN5 anticancer peptide and 5-fluorouracil (5-FU) were taken as a control in all experiments. Circular dichroism revealed an α-helix structure for the peptides derived from PS2Aa1 (P264-G274, Loop1-PS2Aa, and Loop2-PS2Aa) and β-laminar structure for the peptide derived from Alphacoronavirus spike protein Loop1-HCoV-229E. Peptides showed a hemolysis percentage of less than 20% at 100 μM concentration. Besides, peptides exhibited stronger anticancer activity against SW480 and SW620 cells after exposure for 48 h. Likewise, these compounds showed significantly lower toxicity against normal cells CHO-K1. The results suggest that native peptide fragments from Ps2Aa1 may be optimized as a novel potential cancer-therapeutic agents.

Immunomodulation and antioxidant activities as possible trypanocidal and cardioprotective mechanisms of major terpenes from lippia alba essential oils in an experimental model of chronic chagas disease

In the late phase of Trypanosoma cruzi infection, parasite persistence and an exaggerated immune response accompanied by oxidative stress play a crucial role in the genesis of Chronic Chagasic Cardiomyopathy (CCC). Current treatments (Benznidazole (BNZ) and Nifurtimox) can effect only the elimination of the parasite, but are ineffective for late stage treatment and for preventing heart damage and disease progression. In vivo trypanocidal and cardioprotective activity has been reported for Lippia alba essential oils (EOs), ascribed to their two major terpenes, limonene and caryophyllene oxide. To investigate the role of antioxidant and immunomodulatory mechanisms behind these properties, chronic-T. cruzi-infected rats were treated with oral synergistic mixtures of the aforementioned EOs. For this purpose, the EOs were optimized through limonene-enrichment fractioning and by the addition of exogenous caryophyllene oxide (LIMOX) and used alone or in combined therapy with subtherapeutic doses of BNZ (LIMOXBNZ). Clinical, toxicity, inflammatory, oxidative, and parasitological (qPCR) parameters were assessed in cardiac tissue. These therapies demonstrated meaningful antioxidant and immunomodulatory activity on markers involved in CCC pathogenesis (IFN-γ, TNF-α, IL-4, IL-10, and iNOS), which could explain their significant trypanocidal properties and their noteworthy role in preventing, and even reversing, the progression of cardiac damage in chronic Chagas disease.

Potential distribution of four vectors of American Cutaneous Leishmaniasis: The effect of El Niño and La Niña episodes from the perspective of the ecological niche

The potential distribution of four American Cutaneous Leishmaniasis vectors under average conditions during Neutral (in between episodes), El Niño, and La Niña episodes from 2000 to 2018 were constructed through ecological niche modeling. The potential distribution in El Niño and La Niña were compared with the Neutral distribution. The four vector species (Lutzomyia gomezi, Lutzomyia ovallesi, Lutzomyia panamensis, and Lutzomyia trapidoi) decreased the potential distribution with the occurrence of El Niño and La Niña episodes. During El Niño, the reduction was concentrated over the dry ecosystems, while with the occurrence of La Niña, the potential distribution decreased over most of the Neotropic, leaving areas of climatic suitability concentrated in the Andean and Amazon areas, along with some scattered patches. We found evidence that the occurrence of the climatic anomalies has an effect on the potential distribution of this vector species.

Study of methodologies for the formation of a layer that inhibits the formation of bacterial strains in cardiac instruments

Implantable cardiovascular electronic devices are now an essential part of clinical practice in cardiology. The presence in a large number of adult population of problems in the electrical conduction system and several indications with multiple comorbidities has led to an increase in use of implantable cardiovascular electronic devices. However, the application benefit of these devices has been affected by the higher incidence of infections, which represent a serious complication that can lead to significant morbidity, mortality and cost for health centers due to antibiotic treatments, device replacement and longer hospitalization times. For this reason, the present analysis is oriented to possibility of reducing the dissemination of Staphylococcus Aureus by evaluating different techniques for adsorption of a vancomycin antibiotic film in implantable cardiovascular electronic devices. Through this analysis it is possible to establish a new alternative prophylactic method to avoid risks to the health of patients who have problems in the electrical conduction system, which are prone to becoming infected by Staphylococcus Aureus strains during the implantation of the device; in this way, infection can be controlled locally without requiring removal of the initial device and thus avoid an intracardiac compromise that leads to major complications such as endocarditis.

Prevalence of Mycoplasma agalactiae and Toxoplasma gondii in goats and sheep in La Guajira and Cesar, Colombia

The aim of this study was to determine the prevalence of Toxoplasma gondii and Mycoplasma agalactiae in sheep and goats from the south of La Guajira and north of Cesar as part of the establishment of a serological profile of these species in the Colombian region. Serum samples (n=1039) were analysed with commercial ELISA kits specific for these pathogens. The global prevalence found in the region was 25.1 and 18.3% for T. gondii and 0.12 and 0.8% for M. agalactiae in sheep and goats, respectively. Positive samples for T. gondii were more frequent in females than in males. Likewise, all samples positive for M. agalactiae were determined in females.

Methodology for evaluating digital competence of virtual learning master\'s degree students

The primary objective of this study is to develop a methodology for evaluating digital skills in a population of 524 master\'s degree students in virtual learning and a sample of 222 participants at the Universidad de Santander (Colombia). A reliable and validated data collection instrument was used (Cronbach\'s alpha coefficient = 0.987). The approach is quantitative, non-experimental, correlational in scope, and crosssectional. A descriptive-inferential statistical analysis was applied by using a Pearson χ2 correlation test and a principal component analysis with KMO and Bartlett tests. The results show that the relationship between the use and knowledge of information and communication technology (ICT) that the population has, significantly influences the integration of technological resources in teaching practices. It is concluded that evaluating digital competence allows improving pedagogic training of teachers who are master\'s degree students in a virtual learning mode.

Blue rubber bleb nevus syndrome: Presentation of a case and review of the literature

Blue Rubber Bleb Nevus Syndrome, is a rare condition characterized by skin lesions caused by vascular malformations most frequently associated with lesions of the gastrointestinal tract, although rare, it can present with lesions in the central nervous system, thyroid, liver, spleen and lungs; common symptoms are: digestive tract bleeding and iron deficiency anemia. The main manifestation are skin lesions that are characterized by being button-like, with a bluish tint, covered by skin, called blue nevus with a rubbery consistency due to its rubber-like consistency. We present a case of Blue Rubber Bleb Nevus Syndrome with involvement in the central nervous and gastrointestinal systems.

Education in times of climate change for human resilience and environmental regeneration

This article addresses human resilience and environmental regeneration in training the population to adapt and mitigate climate change\'s negative effects. The objective is to let teachers know about these constructs\' implications in the behavior of societies in future climate scenarios and the urgent need for their assessment in the classroom. Constructing the essay implied conducting a thorough review of updated bibliography available in indexed journals and documents of international organizations and its subsequent pedagogical contextualization. The analysis allowed concluding that the school should strengthen, in its students, the biological, cognitive, and emotional processes related to resilience to endure uncertain, vulnerable, and complex environments as a result of the occurrence of extreme temperature events (heat and cold waves) and precipitation (droughts, floods, hurricanes). Likewise, education must train future societies to undertake environmental regeneration processes that rebuild the broken tissues of nature as a mechanism to mitigate and compensate for anthropic damages. In these pedagogical scenarios, the school is an institution of the first order to train citizens who will inhabit a planet with environmental conditions and natural resources still unknown to human beings today. Implementing the analyzed constructs requires educational policies, school curricula, and teachers trained to develop them in their classrooms.

Injury epidemiology in professional ballet: A five-season prospective study of 1596 medical attention injuries and 543 time-loss injuries

Objectives To describe the incidence rate, severity, burden and aetiology of medical attention and time-loss injuries across five consecutive seasons at a professional ballet company. Methods Medical attention injuries, time-loss injuries and dance exposure hours of 123 professional ballet dancers (women: n=66, age: 28.0±8.3 years; men: n=57, age: 27.9±8.5 years) were prospectively recorded between the 2015/2016 and 2019/2020 seasons. Results The incidence rate (per 1000 hours) of medical attention injury was 3.9 (95% CI 3.3 to 4.4) for women and 3.1 (95% CI 2.6 to 3.5) for men. The incidence rate (per 1000 hours) of time-loss injury was 1.2 (95% CI 1.0 to 1.5) for women and 1.1 (95% CI 0.9 to 1.3) for men. First Soloists and Principals experienced between 2.0-2.2 additional medical attention injuries per 1000 hours and 0.9-1.1 additional time-loss injuries per 1000 hours compared with Apprentices (p≤0.025). Further, intraseason differences were observed in medical attention, but not time-loss, injury incidence rates with the highest incidence rates in early (August and September) and late (June) season months. Thirty-five per cent of time-loss injuries resulted in over 28 days of modified dance training. A greater percentage of time-loss injuries were classified as overuse (women: 50%; men: 51%) compared with traumatic (women: 40%; men: 41%). Conclusion This is the first study to report the incidence rate of medical attention and time-loss injuries in professional ballet dancers. Incidence rates differed across company ranks and months, which may inform targeted injury prevention strategies.

Drill design using the ’control-chaos continuum’: Blending science and art during return to sport following knee injury in elite football

Establishing the level of risk, planning and adapting the return to sport (RTS) process following a complex knee injury involves drawing on a combination of relevant high-quality evidence and practitioner experience. On-pitch rehabilitation is a critical element of this process, providing an effective transition from rehabilitation to team training. The ‘control-chaos continuum’ (CCC) is an adaptable framework for on-pitch rehabilitation moving from high control to high chaos, progressively increasing running load demands and incorporating greater perceptual and neurocognitive challenges within sport-specific drills. Drills are a key element of the CCC, and are designed to ensure specificity, ecological validity and maintaining player interest. We showcase drill progression through the phases of the CCC, highlighting the use of constraints to create drills that incorporate the physical, technical, tactical and injury-specific needs of the player. We also provide recommendations to help practitioners create training session content using the CCC to help replicate the demands of team training within their own environment.

Compassion fatigueand related factors in intensive care nurses:A multicentre study

Objective: To determine the levels of compassion fatigue and its Related factors in nursing professionals from three Units Intensive Care. Material and methods: Cross-sectional correlational study carried out between February 2018 and February 2020. A sampling at convenience. The Quality of life questionnaire for professionals ProQOL (Professional Quality of Life Scale) was applied. Results: In general, it was found according to the scale guidelines lower than average levels of compassion satisfaction 45.71% (n = 32), mean levels of Burnout: 56.34% (n = 40) and mean levels of compassion trauma: 49.30% (n = 35). Significant differences were not found in relation to sociodemographic factors. Conclusions: Low levels of satisfaction were found for compassion, which increases the risk of compassion fatigue. No significant differences were found in relation to the years of work experience, age, gender, years of experience, or civil status.

Student Permanence in E-learning Postgraduate Programs: a case Study

The primary objective of this research study is to examine the factors that affect student permanence in a virtual methodology postgraduate academic program at a Colombian university. A literature review on student permanence in higher education programs is performed and a perception exercise is developed and applied to students. The situations that affect academic continuity are determined and strategies are generated to promote student permanence and graduation. The results show that the factors that have the greatest impact on student permanence are personal, socioeconomic, and academic, while technological and institutional factors are less relevant. It is concluded that the motivations that lead students to drop out of their studies are diverse and complex and that different strategies are needed to increase student permanence. This includes strengthening the capabilities of educational institutions to analyze internal and external scenarios to improve education quality and successful completion of academic programs.

Disagreement between PCR and serological diagnosis of Trypanosoma cruzi infection in blood donors from a Colombian endemic region

Introduction: Chagas\' disease is the leading cause of infectious myocarditis worldwide. This infection caused by Trypanosoma cruzi is usually life-long and asymptomatic; however, the third part of infected people can develop severe or even fatal cardiomyopathy. As the parasitemia in the chronic phase is both low-grade and intermittent, T. cruzi infection is principally detected by serology, although this method has sensitivity and specificity limitations. Objective: To determine the level of agreement between serologic and molecular tests in 658 voluntary blood donors from six provinces in the Colombian department of Santander. Materials and methods: We evaluated an array of diagnostic technologies by cross-section sampling performing a serological double diagnostic test for T. cruzi antibody detection (Chagas III ELISA™, BiosChile Group, and ARCHITECT Chagas CMIA™, Abbott), and DNA detection by polymerase chain reaction (PCR). We collected the demographic, clinical, and epidemiological information of participants. The sample size was calculated using Epidat™ and the statistical analysis was done with Stata 12.1™. Results: PCR was six times more sensitive in detecting T. cruzi infection than ELISA/CMIA with prevalence values of 1.8% (12/658) and 0.3% (2/658), respectively, and kappa=0.28 (95%CI: -0.03 - 0.59). In contrast, serology showed a sensitivity of 16.7% (95%CI: 2.09 -48.4) and a specificity of 100% (95%CI: 99.4 - 100). All seropositive samples were found to be positive by PCR. Conclusions: The implementation of PCR as a complementary method for screening donors could reduce the probability of false negative and the consequent risk of transfusional-transmission of Chagas\' disease, especially in endemic regions.

Prospective evaluation of the association between varicocele and benign prostatic hyperplasia in men over 40 years of age

We investigated the association between varicocele and benign prostatic hyperplasia in men over the age of 40 years. A total of 296 outpatients were evaluated. Prostate volume was measured with transrectal ultrasound. Varicocele was diagnosed by physical examination and ultrasound. Prostatic hyperplasia was defined as prostate volume greater than or equal to 40 ml. Two groups were compared: patients with prostate volume less than 40 ml and patients with prostate volume greater than or equal to 40 ml. There was a statistically significant difference between the groups in terms of mean age, post-void residual, International Prostate Symptom Score and PSA. The percentage of patients with clinical varicocele in the group with a volume less than 40 ml and the group with a volume equal to or greater than 40 ml was 38.2% and 47.7% respectively (p =.12). There were no differences between the two groups in the percentage of patients with clinical or subclinical varicocele (43.2% vs. 52.2%, respectively, p =.12). No differences were found in the percentage of patients with varicocele when comparing men with prostates smaller than 40 ml and greater than or equal to 40 ml.
